Re-registration/registration for compulsory health insurance of persons with lower and higher income
Pursuant to the Law on Contributions from Compulsory Social Insurance, the Health Insurance Fund of the Republic of North Macedonia publishes every year at the beginning of February the limits for earned income due to the need for re-registration/registration of persons with low and higher income, in order for them to have continuity in health insurance.
This year, as in previous years, the automatic re-registration of this category of insured persons continues, and we therefore wish to inform persons with low and higher income that there is no need for them to come to the Fund’s regional office to re-register, that is, their health insurance will be extended automatically, with the exception of insured persons who have a change in relation to earned income or who are applying under this basis for the first time.
The new limits for health insurance according to net income earned in 2022 are:
- Up to 216.000,00 denars – health insurance is borne by the state, that is, the Ministry of Health pays the health insurance contribution;
- From 216.000,00 denars to 296.382,00 denars if in the last two months they earned total income of less than 36.000,00 denars – health insurance is borne by the state, that is, the Ministry of Health pays the health insurance contribution;
- From 216.000,00 denars to 296.382,00 denars – health insurance is paid by the insured person in the amount of 50% of the average salary, that is, the person pays a monthly contribution of 1.852,00 denars;
- Over 296.382,00 – health insurance is paid by the insured person in the amount of 50% of the average salary, that is, the person pays a monthly contribution of 1.852,00 denars.
In doing so, we note that the income of a family is considered to include the personal income and the income of the members of the health insurance holder ( spouse, children).
Method of registration/re-registration of persons with low and higher income
Insured persons whose health insurance will be extended automatically and who have no change in relation to the limit of earned income will not receive new decisions.

Persons applying for the first time under one of these bases must come to the counters in the Fund’s regional offices in order to submit an application together with the required documentation, published on the Fund’s website.
