Private Health Institutions (PHIs) providing specialist consultative and/or hospital health care
Notice on the submission of documentation for ensuring the type and scope of health services for insured persons for 2023
All public and private health institutions that have an agreement for the provision of health services in the field of specialist consultative and/or hospital health care at the expense of the Fund’s resources, and for the purpose of ensuring health services for insured persons in 2023, are required to submit the following documentation for 2023:
1. PHIs
- Plan for the type and scope of health services for 2023 by activities – form,
- Summary recapitulation of the total planned health services for 2023 by levels of health care - form,
- List of activities - form,
- List of employees by structure - form,
- Overview of new equipment and new diagnostic procedures for 2023 - form,
- Development components - form.
2. PHIs providing specialist consultative and/or hospital health care (DOES NOT APPLY to PHIs providing primary health care and PHI pharmacies)
- Plan for the type and scope of health services for 2023 by activities,
- Summary recapitulation of the total planned health services, for all health care activities in 2023.
The documentation specified under number 1 must be submitted by all public health institutions, while private health institutions submit only the plan for the type and scope of health services by activities and the summary recapitulation (if they have more than one activity), as specified under ordinal number 2.
The documentation to be submitted should be in accordance with the prescribed templates of the Fund, published on the website www.fzo.org.mk in the section Health Care → Templates of required documentation for 2023.
All documentation must be submitted in:
- electronic form (excel format) by e-mail: [email protected] and
- printed form (verified by the director of the institution), to HIFM - marked for the Department for Budget Coordination and Negotiation.
Note: When submitting the documentation in electronic version, the name of the health institution must be stated in the title of the e-mail, while the content should indicate a contact person, contact telephone number and their e-mail address.
The deadline for submitting the required documentation to the Fund is up to and including 26.11.2022.
